Stefhanie Kauffmann, physiotherapist expert in pelvic floor and co-director of RAP Barcelona, presents the case of a 19-year-old young woman who came to consultation with a complex picture of chronic pelvic pain, vulvodynia, bladder pain syndrome and a very high urinary frequency that intensely conditioned her daily life. The patient reported persistent pain in the vulva, constant feeling of urinary infection despite not having it and needing to urinate between ten and fifteen times a day, which had notably reduced her social life, her bodily comfort and her general well-being.
During the assessment, the team also detected a context of high academic demand and a nervous system clearly in a state of alert, accompanied by muscle hypertonia and postural instability. In response, a combined treatment was proposed that integrated sessions with NESA, individual work in consultation and guidelines for home, with the aim of addressing both the physical and neurophysiological dimensions of the problem.
As Stefhanie explains, after ten sessions the evolution was clearly positive. The pain stopped being persistent, the patient recovered the ability to do physical activity and socialize, returned to wearing clothes that were previously unbearable due to the burning sensation and went from urinating every half hour to doing so every three hours. The case reflects how a comprehensive approach can restore stability, functionality and quality of life in young patients with highly limiting pelvic conditions.
