Rheumatology and Immune System

Chronic inflammation in rheumatology is not always an immune failure: it can be a failure of the autonomic nervous system (ANS) to regulate it. Via the vagal cholinergic anti-inflammatory pathway, the ANS is the primary switch of the immune system. When parasympathetic tone is low, inhibitory control over macrophages, mast cells and T cells is lost: persistent inflammation without clear structural cause.

Non-invasive neuromodulation with NESA XSIGNAL® restores that immune switch, allowing the body to re-establish homeostasis without additional immunosuppression. It does not replace your approach: it enhances it.

Treatment

Non-invasive neuromodulation recalibrates the vagal–sympathetic axis through peripheral stimulation reaching vagal nuclei, synchronising cholinergic signalling in articular macrophages and local immune cells. The result is restoration of the cholinergic anti-inflammatory pathway: pro-inflammatory cytokines decrease, immune tolerance is re-established, neuropathic pain normalises and sleep recovers.

Each session acts as a “reminder” to the immune system of its inherent capacity for self-regulation. Benefits accumulate in reduced pain, normalisation of inflammatory markers and recovery of joint function.

Introducing NESA XSIGNAL®

The impact of regulating the ANS in rheumatology and immune system
Modern rheumatology has mastered the management of structural damage through anti-TNF, anti-IL-6 and other biologics. However, a significant subgroup of patients experience refractory pain and poor drug tolerance with minimal imaging findings. In these cases, the issue is not excessive inflammation, but lost immune regulation. NESA restores that regulation.
The vagus nerve is an immune nerve. Its terminals innervate synovial macrophages, dendritic cells and mast cells. When stimulated, it releases acetylcholine, activating α7 nicotinic receptors in macrophages and suppressing TNF-α, IL-6 and IL-1β. Many patients with refractory inflammation have an inhibited vagus nerve (low parasympathetic tone). Without this “switch”, macrophages remain in a persistent inflammatory state. NESA restores vagal tone, reactivating this immune switch and allowing inflammation to resolve naturally.

Central sensitisation is common in arthritis and fibromyalgia: the central nervous system amplifies local pain signals, resulting in pain disproportionate to the underlying inflammation. This occurs when parasympathetic tone is low and inhibitory control over substance P and NMDA pathways is reduced. The vagus nerve inhibits substance P at the dorsal horn level. Without vagal tone, uncontrolled substance P amplifies nociceptive signalling. NESA restores vagal tone, reactivating these inhibitory mechanisms and normalising pain perception without opioids.

Joint oedema persists when synovial microcirculation is dysfunctional: sympathetic vasospasm combined with impaired parasympathetic lymphatic drainage leads to increased exudation and reduced clearance. Restoring autonomic balance normalises synovial microcirculation, enabling efficient lymphatic drainage and oedema reabsorption without systemic diuretics or local steroids.

Clinical remission in arthritis requires deep NREM sleep. During deep sleep, regulatory T cells activate, IL-10 is produced and local inflammation resolves. Without deep sleep, immune resolution cannot occur. Many arthritic patients experience fragmented sleep due to pain and autonomic dysregulation. NESA restores nocturnal parasympathetic tone, enabling deep, continuous sleep and improved immune control.

Each NESA session provides repeated cholinergic signalling to macrophages and dendritic cells. Over time, this recalibrates the basal immune set point: macrophages become less reactive, regulatory T cells expand and tolerance is restored.
